The Jacob Perlow Hospice, part of the Department of Pain Medicine and Palliative Care, is committed to patients who require specialized care at the end of life.

Services at the Jacob Perlow Hospice include regularly scheduled home visits by team members to evaluate the patient's condition and to supervise care. We provide instructions for families on how to manage physical and emotional symptoms such as pain, anxiety or confusion, and assistance with the personal care of the patient. Our services and support can include the following:

• Bathing and feeding

• Assistance with household chores

• Transportation to and from the inpatient unit

• Supplies

• Equipment and medications related to the primary illness

• Emotional support and counseling for both the patient and the family

• Spiritual counseling

• Short-term care for problems that cannot be managed at home

• Follow-up bereavement counseling for family members after the patient's death
